1.  This data is licensed under an (Attribution-NonCommercial-ShareAlike CC BY-NC-SA), United States Public License, 2011, National Program Office for Public Health Law Research.

As a result you agree to:

1.  Attribute any redistribution or derivative work using the suggested citation on the dataset;

2.  Share-alike any public use, expansion or replication of the data; and

3.  Keep open the data or any adapted version of it.

2.  PHLR makes no guarantee or warranty concerning the accuracy of information contained in the data or metadata including, but not limited to, loss or damage of any kind incurred as a result of the use of any data uploaded, posted, emailed, shared and/or otherwise made available.
